Even with a temporary lift on the shutdown, we know people in our area are still in need —please fill out the form below and we will get in touch with you, and see if Chef's Table Charities can help.

This is a new initiative, so please watch for updates both here, and on our facebook page. 

We need to know more about your individual needs so I can customize assistance boxes. Don’t be afraid to ask. We have lots of people willing to help. Please be as specific as possible so we get you the exact aid you need. The below check boxes  are some of the things we’ve collected, and we can get other things. If your family can use them, wonderful...if not, please leave them for other families in need. We want to make sure that everyone is covered, but we don not want you to hold back and do without!  Please specify if you need men’s or women’s on some on some of the products, make note of that in the comment box below the check list, and we can make sure to try and fill your box with the appropriate items.


    Family In Need : YOUR INFORMATION


    It is important that all of this information is filled out and details provided for you to be considered.  There is absolutely no shame or guilt in asking for help!  This is not something to caused, and there is a whole community out there that wants to help.  Your information will not be shared, used on social media, and discussed without your consent.  Chef's Tables Charities, their friends and partners just want to make sure or community is fed, safe, and secure.

    DUE TO SOME RECENT FRAUD and people trying to take advantage of this program, we are forced to take an extra step in qualifying families and individuals in this application process.  Again, your information will not be shared or used without your consent, but these next steps are REQUIRED without exception.
    If you are a member of the Coast Guard, you will NOT need to do this next step, and we are working closely with them and have the necessary information to proceed.

    If you are with any other entity, the following is REQUIRED and your application cannot be considered without this step.

    Proof of employment is required with application in addition to photo of your current valid Texas Drivers License or State ID. Proof of Employment is an agency or branch ID, pay stub from the last 3 months, or business card.  Further documentation may be required at the discretion of the board. It is best to take a picture of your picture ID with your proof of employment in the same image. You may upload more than one document. Documents may also be emailed to Chefstablecharities@gmail.com. Applications will only be processed upon receipt of all applicable documentation.  No exceptions.

    MUST LIVE IN GALVESTON COUNTY OR GREATER BAY AREA HOUSTON

    Please give us as much information as possible so we can do our best to help out in this time of need.  We have many people willing to step up and help where they can, so please use this list as a guide, but if you have special items you use in your household, don't be afraid to ask.
    If Chef's Table Charities needs more information, the team doing the assessment will be in contact with you via the email address and phone number you provided..  Inquiries about this selection process need to be directed to chefstablecharities@gmail.com, but please be patient with us.
